Position : Whole Person Care Provider

Organization : MyCHN – A nonprofit healthcare leader with 100+ providers and 15+ locations in the Greater Houston and Gulf Coast area

What You’ll Do:

• Diagnose, treat, and manage patient care with autonomy
• Collaborate with physicians & care teams
• Educate and empower patients
• Drive quality outcomes and innovation
• Keep documentation sharp and timely
• Float Providers: Travel between clinics to fill in where needed
• Follows PCMH guidelines and principles, integration of services, and whole person care

What You’ll Need:

• Licensed as Family Nurse Practitioner with recognition as Advanced Practice Nurse or licensed as a Physician Assistant with recognition by the Texas Medical Board
• Master of Science in Nursing
• Certification in area of practice or eligibility for certification
• Must possess or be eligible to obtain a DEA license.
• CPR/BLS Certified. ACLS a plus!
• Bilingual in Spanish? That’s a big plus!
• Advanced clinical skills & prescriptive authority
• Compassion for all communities
• Multitasking mastery & team spirit
