Job Description

Salary: $40,000 - 45,760 per year Requirements:

• We are looking for candidates who are committed to guiding patients through the healthcare system, connecting them to medical services, and providing education and support to encourage engagement and retention in their care. The ideal candidate will use motivational interviewing techniques to promote patient-centered care and enhance medical adherence. Additionally, we value individuals who can help patients overcome barriers to care and improve health outcomes while sharing strategies from personal experience to manage chronic diseases. Participation in our monthly PAWH workshops and activities is also a requirement.

• Engage in ongoing training, supervision, and professional development to enhance skills.
• Take part in case conferences, quality improvement initiatives, and collaborations to represent consumer viewpoints.
• Accompany patients to medical appointments and other health-related services.
• Support patients in navigating and accessing referral sources as needed.
• Assist patients in securing benefits and entitlements.
• Provide education, support, and adherence assistance through individual or group sessions.
• Make reminder calls and follow up on missed appointments.
• Conduct home visits when necessary.
