Job Description

Position Title:  Psychiatric Mental Health Nurse Practitioner (PMHNP) – Outpatient Hybrid/Virtual Care
Location:  Glendale, Arizona (Local Candidates Only)
Schedule:  Full-Time | 3–4 Days per Week
Care Model:  Hybrid – Telehealth and Scheduled In-Person Services
Setting:  Outpatient Behavioral Health
Call Requirements:  None
Travel:  Minimal – In-person availability required on a scheduled basis

Position Overview

A modern, mission-driven outpatient behavioral health clinic in  Glendale, Arizona  is seeking a  Psychiatric Mental Health Nurse Practitioner (PMHNP) to join its expanding care team. This  hybrid role  blends telehealth flexibility with scheduled in-person patient visits, delivering high-impact psychiatric care for adolescents and transitional-age youth.

Core Responsibilities

  • Conduct comprehensive psychiatric evaluations and mental health assessments
  • Develop and monitor individualized treatment plans
  • Prescribe and manage psychotropic medications
  • Provide patient and family education on diagnoses and treatment
  • Collaborate with therapists, case managers, schools, and external providers
  • Participate in clinical team meetings and case reviews
  • Document all services accurately in the electronic health record (EHR)
  • Maintain compliance with legal, ethical, and professional standards
  • Stay informed on best practices in adolescent psychiatry

Qualifications

  • Certified  Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Active, unrestricted  Arizona NP license
  • Minimum 1 year of experience working with adolescents or substance use populations
  • Strong background in trauma-informed and collaborative care models
  • Effective communication and documentation skills
  • Comfortable navigating both virtual and in-person clinical settings
